Remember a while back when Verizon, Alltel, MetroPCS and Sprint said that they would start allowing you to bring over a non-carrier CDMA device to their network? Well it looks like Verizon has actually made good on their promise, unfortunately it will take a minimum of 3 weeks. A forum member over at xda-developers has just posted the “Non-Verizon Wireless Device Activation Request Form” which gives you everything you need to bring that Sprint Touch Pro2 over to Big Red.
The instructions are very simple, send in your phone after filling out all the paperwork and in as little as 3 weeks you will have your phone back in your hands with full compatibility for Verizon’s network. We are sure that plenty of you VZW customers are a bit jealous of the Sprint HTC Hero coming out on October 11 so now might be the time to read up on what it will take to get the device on your network.

The link to this PDF form does not seem to be good anymore. Is there a new link to this form and does verizon have any language regarding this on their website?
There is additional information on PPC Geeks, but I have uploaded the form since that link has stopped working.
